Should You Become Disabled
DISABILITY RETIREMENT PACKAGE
To apply for disability retirement, the forms contained in the Disability Retirement Package must be completed by you, your employer and your medical care providers.
At the same time you submit your Application for Disability Retirement to TRS, you should contact your employer and request that they complete a Retirement Certification Report form. The Application for Disability Retirement and the Member’s List of Disability Information forms should be submitted to our office first, and then the other applicable forms can follow later. However, all forms must be correctly completed and received at our office before we can determine your eligibility for disability retirement.
More detailed information and instructions are included on the forms, so please review all instructions carefully. Also, if you need multiple copies of a form contained in the Disability Retirement Package, the individual forms can be found in the forms drop down menu on the home page of this website.
NOTE: Because TRS does not have any control over the receipt of information from third parties (ie. current or previous employers, doctors, etc.), TRS recommends that you begin soliciting the necessary medical information from these parties before you begin the retirement process. TRS cannot process your retirement application until ALL necessary documentation is received from all necessary parties.
